Are you an experienced Registered Nurse passionate about clinical excellence and mentoring others? We’re seeking a dedicated Clinical Care Coordinator / Nurse Mentor to support and guide a residential aged care facility in a small town, 25 minutes from Adelaide.

About the Role:
As a Clinical Care Coordinator / Nurse Mentor, you will play a vital role in enhancing the delivery of clinical care across multiple aged care facilities. Your expertise and mentorship will help ensure that residents receive high-quality, person-centered care while fostering professional growth and development within the nursing team.

Key Responsibilities:

• Provide guidance and support to nursing staff to uphold the highest standards of aged care.
• Lead and mentor clinical teams, fostering professional growth and best-practice care delivery.
• Ensure facilities meet and exceed accreditation and industry standards.
• Advocate for residents’ well-being and individualized care needs.
• Work closely with Facility Managers, Allied Health teams, and support staff to maintain a positive and compliant care environment.

About You:
To excel in this role, you will bring:

• Current AHPRA registration is essential.
• Experience in a Clinical Care Coordinator, Nurse Mentor, or similar senior nursing role.
• Expertise in aged care, including dementia care and complex health needs.
• Ability to engage effectively with residents, families, staff, and stakeholders.
• Passion for quality care and ongoing professional development.
• Available to work Monday to Fridays
• Knowledge of clinical assessments, care plans, AN-ACC and accreditation compliance and documentation.
