Our Aged Care Program is still in its infancy therefore the Aged Care Manager is a key leadership position within the Services division and is primarily responsible for providing clinical leadership and direction across all service deliveries, ensuring that day to day clinical work is carried out effectively to meet the needs of our clients. This role contributes to the development of evidence-based and innovative clinical services and will promote the delivery of care that is of the highest quality.

The Aged Care Manager will be actively engaged in clinical and caseload reviews and actively manage referrals and demand for clinical services and where required provide direct clinical services.

The Aged Care Manager will be a proactive manager in supporting Holistic Horizons community services financial outcomes to enable the team to maximise funding in different service models, and build a team as the service grows.

This role will suit someone who prefers a hybrid model of mostly working either from home or the office. The candidate will be out in the community a portion of the time.

A Car will be supplied as part of this role.

At least 3 years experience in Managing Home Care Packages
