Assistant Clinical Care Coordinator - Part time

The successful applicants MUST have;

5+ years Aged Care RN experience;

Current AHPRA Registration as a Registered Nurse;

Current police check;

Vaccination certificate;

Greenwood Manor is seeking a skilled & experienced Assistant Clinical Care Coordinator to join our dedicated team, providing clinical leadership and supervising Clinical and Care Staff.

The Assistant Clinical Care Coordinator will work collaboratively with the Facility Manager / Clinical Care Coordinator to provide training & nurturing support to our Nursing & Care teams. We value clear communication, accurate documentation and consistent effort for continuous improvement.

Skills and attributes required:

Excellent verbal and written communication skills.

Strong planning, time management and organisational skills.

Competent in specialised nursing procedures such wound management, diabetes management, palliative care and catheter care and ability to train staff in specialised nursing procedures.

Experience undertaking assessments and formulating Care Plans.

The ability to liaise with doctors, nurses, other health care providers and the pharmacist to facilitate optimum health care.

Infection Prevention and Control qualification (desirable) and experience in Outbreak management.

Experience in Incident Management and SIRS reporting.

An understanding of Medication management including parameters of psychotropic use.
