About the Role

We are seeking a compassionate and experienced Clinical Care Specialist to join our team at NQ Quality Care. This critical role is responsible for coordinating, managing, and delivering high-quality, person-centred clinical care in alignment with NDIS standards and organisational values.

You’ll play a pivotal part in creating care plans, providing clinical oversight, and supporting participants with complex needs. Your ability to collaborate across multidisciplinary teams, advocate for participants, and drive continuous improvement will help shape outstanding outcomes for the people we support.

Key Responsibilities

1. Coordination of Participant Services

Serve as the main point of contact for participants and their families.

Coordinate clinical and support services in line with NDIS plans and participant goals.

Monitor progress and adjust care plans to reflect changing needs.

Ensure smooth service transitions with effective advocacy and communication.

2. Care Planning & Implementation

Develop, implement, and review individualised care plans.

Deliver direct support where needed, especially in complex cases.

Ensure team understanding and execution of care plans.

3. Clinical Oversight & Support

Provide clinical guidance across the care team.

Conduct assessments, analyse outcomes, and initiate quality improvements.

Maintain accurate and compliant clinical documentation.

4. Compliance & Risk Management

Ensure services meet NDIS and regulatory requirements.

Proactively identify and manage risks to participant safety and well-being.

Assist in audits, incident reporting, and compliance reviews.

5. Stakeholder Engagement & Collaboration

Build strong, respectful relationships with participants, families, and external partners.

Lead and participate in case meetings and care conferences.

Advocate for participant needs and service accessibility.

6. Training & Staff Development

Train and mentor staff in clinical best practices and person-centred care.

Support ongoing learning and development across the team.

Assist in the creation and delivery of internal training programs.

7. Clinical Documentation & Reporting

Ensure documentation is accurate, up-to-date, and regulatory-compliant.

Prepare reports on participant progress and service delivery.

Contribute to policy development and clinical procedures.

Key Selection Criteria

Qualifications & Experience

Tertiary qualification in Nursing or Allied Health (e.g., OT, Physiotherapy, Speech Pathology).

Current registration with AHPRA or other relevant professional bodies.

Minimum 3 years’ experience in clinical care or coordination; NDIS experience highly regarded.

Demonstrated experience in care planning for complex needs.

Skills & Knowledge

Strong knowledge of NDIS frameworks and standards.

Advanced clinical assessment and care planning skills.

Person-centered care expertise with excellent communication and collaboration abilities.

Proficiency in clinical documentation systems and technology.

Working Conditions & Requirements

Office-based role with travel to participant homes or service locations as needed.

Ability to perform physical tasks related to participant care including Manual Handling.

Must hold:

Proof of Working Rights

First Aid and CPR Certificate

Valid Driver's Licence

Current AHPRA registration

NDIS Worker Screening Card

Working with Children Check (Blue Card)
