About the Role
We are seeking a passionate and experienced Coordinator of Supports (CoS) to join our dedicated team. This role involves supporting NDIS participants to navigate the NDIS system, connect with services, and build capacity to manage their own plans over time.

Key Responsibilities

Support participants to understand and implement their NDIS plan

Connect participants with appropriate community, mainstream, and funded supports

Monitor progress toward achieving goals and adjust supports where required

Work collaboratively with participants, families, providers, and community stakeholders

Ensure all services meet the requirements of the NDIS Practice Standards

Maintain accurate documentation, reporting, and case notes

What We're Looking For

Previous experience as a Support Coordinator or in a similar role within the disability, community, or aged care sector

Strong understanding of the NDIS framework and person-centred practice

Excellent communication, interpersonal and problem-solving skills

Ability to work independently and manage a caseload effectively

Current Driver’s Licence and reliable vehicle

Relevant qualifications in Social Work, Community Services, Disability or equivalent

Working with Children Check, NDIS Worker Screening Check, and Police Clearance
