About the Role
To provide leadership and guidance for Support Worker staff to achieve positive and quality outcomes for the people CLO supports. Staff supervision is a key aspect of this position. The Service Coordinator is required to actively support employee development to support overall participant outcomes and maximise their independence.
Key Responsibilities
• Participate in the development and review of participant plans and programs
• Monitor and assess plans and plan implementation for effectiveness to ensure outcomes are met, including meeting goals.
• Negotiates service offerings within boundaries.
• Ensures personalised development plans are in-effect for team members.
• Leads active support.
• Ensures supports are effectively and properly documented in accordance with funding requirements.
• Roster design and innovation that fits within funding framework.
• Forward planning of rosters and risk management in establishing suitable workforce management practices.
• Monitor and report on rostering costs, and facilitate activities within the budget.
• Approve timesheets in accordance with payroll processes to ensure accurate of payment of wages and entitlements.
• Approve leave, ensuring coverage.
• Run reports to support workforce analysis.
• Strong communication and reporting to key stakeholders.
• Completion of internal and external incident management reporting/investigations.
To be Successful
• Experience with development and delivery of complex service plans and projects
• Ability to always apply and demonstrate a flexible and person-centred approach, assisting participants to design and adapt homely environments to achieve individualised quality of life.
• Develops and maintains effective stakeholder relationships.
• Leads, guides, and educates staff in effective reporting, documenting and administration and documents supervision and coaching.
• Educates and develops others to achieve quality developments
• Guides therapeutic interventions.
• Preferred understanding of projects and mental health/hospital discharge planning and pathways.
• Advanced proficiencies in Microsoft.
Qualification and Essential Licences
Current Drivers Licence
NDIS Worker Screening Check or willing to obtain
Working with Children Check or willing to obtain
First Aid and CPR certificate
Tertiary qualification in Disability, Social Work, Behavioural Science, Psychology, or similar/equivalent
Satisfy all Visa requirements to work in Australia
