About the role

Limitless Community Services PTY LTD' is seeking an experienced Services Coordinator to join our team in Cockburn Central, WA. This full-time role will be responsible for coordinating and overseeing the delivery of high-quality disability support services to our valued clients within the local community.

What you'll be doing

Developing and implementing service plans to meet the individual needs of clients

Scheduling and coordinating support workers to ensure seamless service delivery

Monitoring and evaluating service quality to ensure client satisfaction

Providing leadership and support to a team of dedicated support workers

Maintaining accurate client records and reporting on key performance indicators

Collaborating with other healthcare professionals to deliver holistic support

Identifying opportunities for service improvements and implementing change

What we're looking for

Relevant tertiary qualification in Community Services, Disability Studies or a related field

Minimum 2 years' experience in a similar services coordination or case management role

Strong understanding of the aged and disability support sectors and relevant legislation

Excellent communication, interpersonal and organisational skills

Demonstrated ability to lead and motivate a team of support workers

Proficient in using client management systems and data reporting

Passion for making a positive difference in the lives of vulnerable community members
