Partner In Your Care (PIYC) Service is a community-based disability service provider registered under National Disability Insurance Scheme (NDIS). Partner In Your Care provides range of services to people with psychosocial disability from daily activities and community participation. A range of services that enable our clients to feel more inclusive, self-dependent and participate within society.

About the role:

The Team Leader is primarily responsible for leading and collaborating with team members to provide duty of care and coordinate the daily operations of a Supported Independent Living (SIL) service to the participants. This role involves a range of responsibilities but is not limited to providing personal care using a person-centered care model, emotional support, and practical support to assist our participants achieve their goals and improve their quality of life.

You will be responsible for:

Working within and uphold the mission, vision and values of Partner In Your Care (PIYC)

Providing a personal centered delivery service to participants.

Building the Support Worker’s capabilities and a highly performing SIL team through support and mentoring.

Establishing, building, and maintaining positive relationships with participants, their families, and stakeholders.

Completing required documentation and administrative tasks in required time frames.

Actively contributes and participates as part of an integrated services team.

Contributes to continuous quality and service delivery improvement throughout the organization.

Ensuring mandatory licenses / competencies are always valid.

Acting in accordance with Work Health & Safety policies and procedures at all times.

Participating in staff meetings and staff training requirements as required.

Preparing any relevant reports for Operational Manager.

About you:

To be successful, you must have:

Bachelor or tertiary qualifications in Aged Care, and/or Disability, Social Work, Mental Health or other relevant qualifications

Proven leadership experience with the ability to coach, lead and mentor a team

Strong passion for delivering a quality person centered service to participants

Ability to influence others, including the ability to educate, coach, counsel and lead staff

NDIS Worker Screening Check

Working with Children Check

First Aid & CPR Certificate

NDIS Module Certificates

Must have a valid Australian driver’s license

A vehicle with fully comprehensive car insurance.

Communicate in an additional language including AUSLAN (Preferred)
