The Exciting Opportunity

• Client-Focused Coordination: Oversee a caseload of Home Care Package (HCP) clients, ensuring quality service delivery, risk management, and compliance with funding guidelines
• Holistic Assessments: Undertake comprehensive assessments, update care plans regularly, and maintain ongoing communication with clients, families and service providers
• Compliance & Quality: Maintain a strong understanding of the Aged Care Quality Standards and Commonwealth legislation, ensuring best practice across all aspects of service
• Team Engagement: Contribute to the team’s success by occasionally participating in the on-call roster, sharing your knowledge and providing support

About You

• Certificate III in Individual Support (Ageing) is essential
• Proven experience in HCP case management including care planning, assessments, risk identification and budgeting
• Familiarity with aged care legislation and the MyAgedCare portal is a must
• Excellent written and verbal communication skills and able to thrive in a fast-paced environment while managing a complex caseload
• Willing to travel across Perth's Northern and Eastern suburbs
