This Position requires :
Previous Experience in Aged Care Home Care Packages
Experience in Managing service inquiries and referrals
Experience in scheduling of staff to meet the needs of our clients and their referring organisations
Work effectively with clients, their families, and other stakeholders to deliver high-quality, person-centred care.
Experience validating time sheets, and compiling requisite documentation to guarantee high-quality and efficient service provision.
Monitor client’s budgets and ensure that they comply with organisation's requirement
Experience in developing care plans and agreements in collaboration with clients and stakeholders.
Monitor and evaluate service delivery through quality improvement measures.
Manage client issues and respond to feedback effectively.
Liaise and work collaboratively with key stakeholders to establish a mutual goal of coordinating service delivery and developing effective linkages relevant to the achievement of the individuals HCP goals
Ability to effectively engage in communication in complex multi-disciplinary teams and services to support HCP participants.
Develop and establish effective professional working relationships with community organisations (mainstream and disability) that enable the delivery of client outcomes
Qualifications & Experience:
Preferably 1.5 Years of Minimum Experience in this role or similar
Degree qualification in Nursing, Allied Health, or Social Work desirable.
Experienced with care planning, budgets, and assessments
Positive, flexible, and dynamic personality
Experience with Engaging with Stakeholders and Ability to Adapt
