Community-Based Registered Nurse – Perth

We’re a locally-run home care organisation dedicated to supporting older adults to live safely and comfortably in their own homes across Perth.

Due to ongoing growth and demand for our services, we are expanding our clinical care team and looking for a dedicated Community Registered Nurse to join us.

About the Role

You’ll deliver essential clinical services to older adults living at home, with a strong focus on wound care, catheter care, risk management, and chronic disease management. You'll work closely with the Clinical Nurse Manager and Care Managers to ensure the best outcomes for our Clients.

Full and Part Time Opportunities Available

Key Responsibilities include:

Conducting comprehensive clinical assessments

Providing wound and catheter care

Managing chronic conditions and risk assessment

Liaising with clients, families, GPs, allied health, and other service providers

Collaborating regularly with the Clinical Nurse Manager and Care Managers to review and tailor care plans

About You

We are looking for an experienced, compassionate, and reliable Registered Nurse who is confident working independently in the community and wants to make a meaningful difference in people’s lives.

To be successful, you will have:

Current AHPRA Registration (with no notations)

Strong experience in wound care and catheter care

Knowledge of Aged Care Quality Standards

Excellent clinical assessment and problem-solving skills

A caring, empathetic nature and strong communication skills (both verbal and written)

Current WA Driver’s License and reliable vehicle

Current National Police Clearance (or willingness to obtain)
