About the role
We’re always excited to welcome new Great Mates to our rapidly growing team. We currently have multiple great opportunities for experienced and dedicated Community Nurse (Registered Nurse). This is an exciting and rewarding role where you’ll be working a 7-day week roster across several locations in the Gold Coast region as per the needs of the people we support. You’ll be responsible for bringing care and support to our participants through dedicated clinical nursing services, as well as supporting the internal Great Mates team as a key member. About The Position:
Participate in providing comprehensive and direct nursing care
Provide individualised case management to specific group of clients in a specific area of nursing practice
Address and respond to changes in client conditions
Collaborate with other health professionals such as General Practitioners (GP) and Allied Health to ensure appropriate actions are taken based on client needs
Work with other healthcare professionals to ensure client needs are understood and planned for
Assist with client care tasks such as care planning and reviewing information used by lifestyle assistants
Encourage the use of technology to improve client services and outcomes
Provide chronic disease and health education/coaching to clients and their families, considering cultural, spiritual, intellectual/educational, and psychosocial differences
Ensure documentation meets quality and compliance standards
About You:
A minimum of 2 years of experience and a current AHPRA registration
Strong knowledge and experience in managing chronic diseases
Demonstrated ability to use professional judgement and gather information during decision-making processes
Familiarity with community nursing services and willingness to learn new technologies
Strong communication and administration skills
Current Open Drivers License (with no restrictions).

Provide evidence of immunisation history
