As we continue to expand, we are seeking an experienced and dedicated Registered Nurse (RN) / Enrolled Nurse (EN) to join our dynamic team as a Community Nurse. As a Registered or Enrolled Community Nurse, You will:
Deliver clinical care to NDIS participants in the community, following individual care plans and assessing client needs.
Provide ongoing health assessments, monitoring and management of chronic conditions, and support for participants’ daily health needs.
Develop and implement nursing interventions, ensuring the highest quality of care and compliance with NDIS standards.
Collaborate with participants, their families, and other healthcare professionals to ensure a holistic approach to care.
Respond promptly and effectively to changes in a participant’s health and wellbeing, initiating referrals and adjustments to care plans as needed.
Maintain accurate, up-to-date records and report on clinical outcomes and participant progress.
To Be Successful in this role, You Will Need:
Current AHPRA registration as a Registered Nurse (RN) / Enrolled Nurse (EN) with no restrictions.
Relevant Nationally recognised nursing qualification.
Experience working in the disability sector, preferably with NDIS participants.
Strong clinical assessment, communication, and interpersonal skills.
Ability to work independently and as part of a team, with a commitment to person-centred care.
A valid driver’s licence and reliable transport to travel within the community.
NDIS Clearance Check
Ability to work flexibly across a range of shifts, including evenings and weekends.
