About the role
The Disability Client Coordinator of Supports In Sync is responsible for overseeing and providing exceptional customer service to our clients of Supports In Sync. You will be responsible for our portfolio of clients.
The Disability Client Coordinator will work closely with other stakeholders to develop and implement strategic plans to achieve client satisfaction and outcomes. This full-time position is based in Rivervale, WA 6103, and will involve coordinating and overseeing the delivery of disability services to clients in the local community.
What you'll be doing
Manage the day-to-day operations of Client Services
Developing and maintaining positive relationships with NDIS participants who are clients of Supports In Sync
Developing and maintaining positive relationships with other stakeholders such as the Client’s families, Support Workers, Allied Health Professionals, General Practitioners, NDIS Care employees and other community services.
Link the clients with other government agencies, services and other stakeholders including but not limited to employment services, housing, Allied Health Professionals to maximise the best outcome for the client if needed.
Liaise with internal clients such as the Service Coordinator, Operations Coordinator, Business Development Coordinator, Human Resources, and the Director
NDIS Reporting in a timely manner (6 weeks from service agreements expiration dates) and sending off to the participants Service Coordinator or Psychological Recovery Coach
Assist in resolving clients’ issues by listening to complaints and offering the solutions effective for satisfactory resolution
Maintain excellent client service and ensure the service team including the support workers are up to acceptable standards to achieve the best outcomes by way of support plans for Support workers.
Communicate with clients to identify their preferences and facilitate the process to meet their needs and expectations
Actioning client enquiries in a timely manner
Compliance with code of conduct, policies and procedures and relevant government legislation and standards where relevant and keeping up to date with any changes
Report incidences to NDIS/Aged Care in line with their policies, procedures, and legislation
Work with the team across all levels to mitigate risks
Ensure appropriate use of resources
Actively participate in initiatives to maintain, build upon, and promote a positive and collaborative workplace
Identify opportunities to integrate and work collaboratively across teams
Provide clients with regular updates and inform them of any change
Carry out research to evaluate customer satisfaction level and discover better techniques for ensuring a satisfied customer base.
What we're looking for
Diploma, Advanced Diploma or Bachelors of Community Services, Allied Health, or a related field
2+ years of experience in client services or a related field
2+ years’ experience with the NDIS Framework
Strong leadership and management skills
Excellent communication and interpersonal skills
Ability to work well under pressure and meet tight deadlines
Strong problem-solving and decision-making skills
Excellent verbal and written communication skills
Strong time management and ability to prioritise
Excellent attention to detail with a resourceful 'can do attitude'
Ability to use Microsoft Office suite of programs and other computer systems
Excellent problem-solving skills
Ability to work in an autonomous and confidential manner
Experience in a related industry is a requirement (Disability/Aged Care or Health Care/Clinical Background
