The Care Partner ensures that consumers receive high-quality care services that meet their individual needs and preferences. This involves coordinating and overseeing all aspects of a consumers’ care plan, from initial assessment and development of the care plan to ongoing monitoring and adjustment of the plan as needed.
The Care Partner is responsible for ensuring that all services are delivered in a safe, effective, and timely manner, while also ensuring that the consumers’ rights and dignity are respected. Works closely with the consumer, their family members, and other healthcare professionals involved in their care, to ensure that all aspects of the care plan are being implemented as intended.
Clinical Care Partners (those with AHPRA registration as an EN or RN), are also responsible completing onboarding documentation and updating/maintaining the full scope of client documentation.
Business Management
Conducts initial assessments of clients to determine their care needs and preferences. Develops comprehensive care plans that address all aspects of the consumer's physical, emotional, and social needs. Coordinates and organizes clients necessary care services, such as nursing, personal care, domestic assistance, allied health therapies, and transportation. Ensures that all care services are delivered by qualified and trained healthcare professionals. Monitors and evaluates the consumer's progress and adjusts documentation as needed. Ensures that all documentation related to the consumer's care is accurate and up-to-date. Advocates for the consumer's needs and rights and ensures that they are receiving the best possible care.
Work Health and Safety
Ensures that the workplace is safe and healthy for all employees, consumers, and visitors.
Identifies potential hazards and implements measures to mitigate or eliminate them.
Provides education and training to employees on safe work practices and procedures.
Conducts workplace risk assessments and recommends appropriate control measures.
Investigates incidents and accidents that occur in the workplace and implements measures to prevent them from happening again.
Ensures that all equipment and supplies used in the workplace are safe and maintained properly.
Keeps up to date with relevant legislation, regulations, and guidelines related to workplace health and safety.
Business Development
Builds relationships with potential consumers. Works with other healthcare providers or organizations to build and maintain partnerships that can help the organization grow and develop.
Operations
Works with other members of the team to coordinate care services for consumers and ensure that their needs are being met. Manages the organization's resources, including staffing, equipment, and supplies, to ensure that they are being used effectively and efficiently. Monitors the quality of care being delivered and implements measures to improve it, as needed. Responsible for managing consumer’s budgets.
Commitment to Continuous Quality Improvement
Responsible for conducting regular consumer satisfaction surveys to gather feedback on the quality-of-care services and identify areas for improvement.
Analyses consumer feedback and work with the care team to develop and implement strategies to address areas for improvement.
Monitors and evaluates the effectiveness of care plans and identify opportunities for improvement.
Ensures compliance with relevant standards and regulations related to the delivery of care services.
Participates in quality improvement activities such as audits, reviews, and evaluations to identify areas for improvement.
Professional Conduct
Maintains confidentiality of consumer information and ensures that all staff members do the same.
Ensures that all care team members adhere to ethical principles in the delivery of care services, such as respecting consumer autonomy, informed consent, and confidentiality.
Ensure that all care team members comply with relevant regulations and standards related to the delivery of care services.
Ensure that all care team members maintain professional boundaries with consumers and their families.
Promotes a culture of professionalism among care team members, encouraging respectful communication, teamwork, and ongoing education and training.
Addresses professional misconduct or ethical violations by care team members, ensuring that appropriate corrective actions are taken.
Professional Development
Encourages care team members to engage in ongoing learning and professional development, such as attending conferences or pursuing advanced certifications.
Participates in performance appraisals of support workers to assess their knowledge and skills, and to ensure they have the necessary competencies to provide high-quality care services.
Evaluates the effectiveness of training and education programs and make improvements as needed.
Provides feedback and recognition to care team members who demonstrate high levels of competency and excellence in their work.
Selection Criteria
Ability to build and maintain relationships with consumers, identify their needs and preferences, and persuasively communicate how our services can meet their requirements.
Manage multiple tasks and meet deadlines in a fast-paced environment.
Strong problem-solving and decision-making skills to address complex issues and make informed decisions in a timely manner.
Excellent interpersonal communication skills.
Knowledge, Experience and Skills
A degree or diploma in Nursing or a Diploma in Community Services.
A minimum of 2 years of experience in a similar role is preferable
Understanding of relevant legislation, policies and guidelines governing the provision of home care services, such as the Aged Care Quality Standards, Home Care Common Standards, and the National Disability Insurance Scheme.
Demonstrated advanced computer skills including Microsoft Office 365 and various other software applications.
Probity Requirements
A National Criminal Clearance Check less than 6 months old prior to engagement: and subsequent criminal records checks at least once every three years.
Working with Children Check or willing to obtain.
NDIS Screening check or willing to obtain.
CPR and First Aid Certificate or willing to obtain.
The Right to Work in Australia.
