Salary Range: $44.58 – $46.93 per hour + super + travel reimbursement
About the Role
At Focused Health Care, we are seeking compassionate and motivated Care Partner / Enrolled Nurse to join our team, supporting older people and people with disability to independently and with dignity in their own homes.
This is a dual care management and clinical role that blends person-centred care with hands-on nursing support under the new Support at Home Program and upcoming Aged Care Act reforms.
As a Care Partner, you will be the key contact for participants — developing care plans, coordinating services, and mentoring Support Workers and Restorative Care Partners.
As an Enrolled Nurse, you’ll provide safe, high-quality nursing care under the supervision of a Registered Nurse, ensuring continuity and clinical oversight across community settings.
Key Responsibilities
Clinical Care and Collaboration
• Deliver safe and effective nursing care in accordance with the Medicines and Poisons Act 2019.
• Monitor participant health, escalate clinical concerns, and collaborate with Registered Nurses and Allied Health Professionals.
• Support participants within the Restorative and End-of-Life pathways, ensuring care reflects comfort, dignity, and clinical best practice.
Care Planning and Coordination
• Partner with participants to develop and review personalised care plans that reflect their goals, preferences, and cultural needs.
• Coordinate services across FHC’s aged care, disability, and restorative programs to support independence and wellbeing.
• Monitor progress, identify changing needs, and adjust plans accordingly to achieve quality outcomes.
Team Leadership and Support
•Supervise and mentor Care Partners and Support Workers, promoting a positive, values-based workplace culture.
•Lead by example in providing person-centred, rights-based, and culturally safe care.
•Support training, onboarding, and continuous improvement initiatives.
Compliance and Continuous Improvement
• Ensure all care aligns with the Strengthened Aged Care Quality Standards, NDIS Quality and Safeguarding Framework, and internal policy.
• Maintain accurate and timely documentation, budgets, and reporting.
• Identify and manage risks to ensure the safety and wellbeing of participants and staff.
About You
You are a caring, adaptable professional who enjoys supporting others to live well at home. You bring strong coordination, communication, and leadership skills, and thrive in a dynamic, person-centred environment.
Essential Criteria
• Diploma of Nursing and current AHPRA registration (Enrolled Nurse).
• Experience in community care, aged care, or disability services.
• Demonstrated ability to coordinate and manage individualised care plans.
• Strong leadership and mentoring skills for supporting frontline staff.
• Excellent communication, documentation, and time management skills.
• Understanding of the Aged Care Quality Standards, NDIS Quality & Safeguarding Framework, and Work Health & Safety principles.
• Competence with Microsoft Office and care management systems (CRM).
Compliance Requirements
• Current Queensland Driver’s Licence and reliable vehicle.
• Current First Aid and CPR Certificate.
• Working with Children Check (Blue Card) and NDIS Worker Screening (Yellow Card).
• Aged Care National Police Check.
• Fully vaccinated against COVID-19 and other vaccinations as per FHC’s policy.
Desirable
•Experience supporting people with dementia, complex needs, or palliative care.
•Training in trauma-informed care, cultural safety, or restorative practice.
•Familiarity with My Aged Care and digital case management systems.
