Clinical Care Coordinator - Full Time

Illawarra Diggers is seeking to appoint two Clinical Care Coordinators (Registered Nurses) ideally with experience in Aged Care, to join our dedicated team.

A minimum of 3-5 years of aged care experience is required for this position.

The Clinical Care Coordinators (CCCs) will report to the Residential Manager and manage the RNs, Team Leader and Personal Care Workers.

The CCCs will be responsible for leading, managing and mentoring the clinal and care staff to deliver high quality care to our 105 residents.

The CCCs will cover a range of leadership duties such as: review client incidents; ensure SIRs are duly recognised, reported and investigated; ensure validated assessment tools are completely promptly; ensure accurate and effective documentation; support staff with clinical decisions; work with allied health professionals and GPs to ensure holistic care for residents.

To be considered you will have:

Current registration with the Australian Health Practitioner Regulation Agency(AHPRA)

Exceptional clinical leadership skills and knowledge.

Demonstrated experience in a similar role or relevant senior RN experience

Experience in aged care coordination roles will be considered as acceptable similar role experience.

A passion for working with older people

Ability to work in a professional team environment

A high level of written & oral communication and computer literacy

Proficiency in advanced clinical software systems is essential for electronic documentation and patient management.

Experience with any aged care software system will be considered suitable for this requirement.

Strong interpersonal skills

Current First Aid Certificate

Ability to work rostered shifts and weekends

Valid Australian work rights or appropriate work visa required.

It will also be an advantage to have:

Palliative experience

Wound management experience

Infection control management experience

Knowledge and experience in the aged care accreditation process.
