The Opportunity
Join a purpose-driven government organisation that provides essential services to Queenslanders living with serious injuries. As a Senior Support Planner, you’ll play a key role in delivering person-centred planning and care coordination that empowers participants to achieve independence, inclusion, and improved life outcomes.

Key Responsibilities

• Develop, review and implement participant support plans in line with legislative and policy requirements.
  
  
• Make informed decisions and recommendations about treatment, care and support needs.
  
  
• Provide expert advice and mentoring to less senior team members.
  
  
• Collaborate with internal and external stakeholders to ensure effective service delivery.
  
  
• Identify opportunities for continuous improvement in support planning practices.
  
  

About You
You’re an experienced professional with strong capability in support planning, case management, or statutory decision-making—ideally within disability, health, or injury management contexts. You bring sound judgement, empathy, and a solutions-focused approach to complex participant needs.

What You’ll Bring

• Proven experience in support planning, coordination of services, or assessing treatment and care needs for people with serious injuries or disability.
  
  
• Strong communication and stakeholder engagement skills.
  
  
• Ability to lead, mentor and provide technical guidance to others.
  
  
• Tertiary qualifications in human services, allied health or a related discipline (highly regarded).
  
  
• Current Queensland Blue Card (or ability to obtain).
  
  
• Open Queensland driver’s licence (for occasional travel).
