We’re seeking an experienced Mental Health Professional to join our multidisciplinary team (MDT) at Wunan Health and be a part of a new and innovative initiative aiming to strengthen integrated and coordinated health care across our health system.

Wunan Health and Wellbeing Centre is a purpose-driven social enterprise committed to supporting the health and wellbeing needs of all our community. Our primary health care services include an accredited private GP clinic, integrated with community services in youth mental health (headspace), disability advocacy and our new residential AOD withdrawal service (KWIC).
About the role
In this rewarding role, you’ll deliver responsive, trauma-informed, strengths-based counselling and focused psychological supports for individuals living with chronic health conditions. You’ll be part of our growing MDT, working closely with GPs, AHPs, RNs allied health professionals, and community partners to deliver holistic, person-centred, care.
Key responsibilities
- Deliver short-term individual and / or group-based evidence-based counselling and / or focused psychological strategies within a case-work framework
- Conduct assessments and develop care and safety plans in collaboration with service users, carers and / or families in line with their GP management or treatment plans
- Participate in multidisciplinary meetings, joint care / case conferencing, and collaborative care planning
- Work collaboratively and support integrated service pathways with local mental health and AOD providers, visiting specialists, and other community stakeholders
- Provide outreach counselling with partner services or home visits where needed
- Support professional development activities, supervision, and / or mentoring of less experienced staff or students (as required)
- Contribute to mental health related continuous quality improvement, clinical governance, and compliance initiatives

About you
- Tertiary qualification in psychology, social work (mental health accredited), occupational therapy, mental health nursing, or Aboriginal and/or Torres Strait Islander Primary Health Care Practice (Cert IV with mental health qualifications)
- Current registration with AHPRA or relevant professional body
- Minimum 2 years’ counselling experience
- Exceptional team-player with excellent communication and interpersonal skills
- Commitment to strengths-based, responsive, person-centred care empowering individuals to live well with chronic conditions
- Sound understanding of mental health (case-management) workflows (referral, intake, care planning, risk assessment and planning, discharge)
- Ability to navigate integrated IT systems confidently and proficient in accurate data collection and report writing;
- Eligible for Medicare billing (highly desirable)
- Able to commence late December 2025
