About the role:
We are looking for compassionate and dedicated professionals to provide specialised support, helping people navigate the complexities of aged care and connect with essential community services through our Care Finder Program.
As a Care Finder, you will play a key role in:
Guiding individuals through the process of understanding and accessing aged care supports.
Connecting people with relevant community services and supports tailored to their needs.
Improving outcomes for our clients by enhancing coordination between health, aged care, and other local systems.
Bridging gaps in understanding and accessibility, ensuring seamless integration of services at the local level.
About You:
Qualification in Certificate III or IV in community services, aged care or related field desirable
Ability to establish and maintain effective relationships with clients and the community
Knowledge of the My Aged Care process and related service providers.
