Your opportunity:

• To deliver high level occupational therapy clinical services and professional expertise to patients identified for the rapid access geriatrician-led model of care.
• Work autonomously within the multidisciplinary team to manage, coordinate and provide optimal, responsive and flexible Occupational Therapy services in acute, community, and virtual settings.
• This position will focus on preventing inappropriate or avoidable admissions by supporting the needs of the patient, their family, and significant others through sub-acute supports in the home environment.
• This role is a short sub-acute service (up to 14 days) to provide intervention in the home environment to reduce the need for hospitalisation and facilitate reablement through improvements to function in the home environment.

Temporary Part Time (53.2 hours p.f.) from date of appointment for up to 12 months with the possibility of extension

Our offer:

You will receive a competitive remuneration package value up to $110,152 p.a., comprising salary rates $64.64 - $69.56 p.h., employer contribution to superannuation (up to 12.75%) and annual leave loading (17.5%) (HP4) (Temporary Part Time, 53.2 hours p.f., from date of appointment for up to 12 months with the possibility of extension).
