We are seeking an experienced Care Manager to join a close-knit team in Kandos, overseeing a fully accredited Aged Care facility. In this role, you will work closely with the Facility Manager to oversee all aspects of clinical care, drive continuous improvement, and support your team to deliver exceptional outcomes for residents.

🧠 What You’ll Be Doing

• Lead and manage the clinical care team to ensure excellence in care delivery
• Support compliance with the Aged Care Quality Standards
• Mentor and develop clinical staff, including RNs and Carers
• Collaborate with families and external stakeholders to support resident wellbeing
• Manage care plans, assessments, and risk mitigation strategies

✅ About You

• AHPRA Registered Nurse with strong clinical leadership experience
• Previous experience in a Care Manager or senior clinical role
• Excellent understanding of Aged Care Quality Standards and accreditation
• Compassionate leadership style with a focus on team development
• Committed to delivering high-quality, person-centred care
