About the role

As a Clinical Care Coordinator at Villa Del Sole', you will play a vital role in ensuring high-quality, personalised care for our residents. This full-time position is located in Glenroy, VIC and will see you working closely with our nursing and care team to coordinate the clinical needs of our aged care community.

What you'll be doing

Assessing, planning, implementing and evaluating clinical care for our residents

Coordinating and overseeing the delivery of nursing and personal care services

Ensuring compliance with all relevant aged care standards and regulations

Maintaining detailed clinical records and providing reports as required

Participating in the development and review of care plans

Providing mentorship and support to our nursing and care staff

Collaborating with other healthcare professionals to deliver integrated care

What we're looking for

Registered Nurse (Division 1) with a current AHPRA registration

Minimum 5 years' experience in aged care nursing, with demonstrated expertise in clinical coordination and care planning

Strong clinical assessment, problem-solving and decision-making skills

Excellent communication and interpersonal skills, with the ability to work collaboratively in a team

Highly organised with attention to detail and the ability to prioritise effectively

Commitment to providing the highest standard of resident-centred care
