About the Role:
The Aged Care Lead will be expected to liaise and support our team and other healthcare providers to deliver positive and meaningful outcomes for the elderly members of our community. You will be responsible for assessing clients needs in a compassionate and caring way that ensures individual support programs are achieved.
You will work closely with the support team of the resident including family members to ensure they have a dignified, safe and care environment.
Key Responsibilities:
Assessment : Assessing the individual needs of residents and ensuring appropriate access to services.
Monitoring: Regular monitoring of the agreed support programs to ensure they are meaningful and appropriate.
Reporting: Completing all reporting including care plans in a timely manner, including maintaining accurate records and documentation.
Supporting: Providing emotional care and support to the resident to ensure their physical and mental health needs are understood and met.
Business Development: Ensuring we are always developing our business further.
Champion person-centred assessments that recognise each resident’s story and needs.
Foster strong partnerships with families, teams and healthcare providers.
Drive innovation in how we deliver quality aged care services.

Skills and Experience:
Education: Bachelor’s degree in Aged Care, or a related field.
Experience: Proven experience as an Aged Care Lead or similar role within a service provider industry.
Industry Knowledge: Experience in the aged care sector is essential. Working understanding of your ‘duty of care’ in delivering services and appropriate reporting requirements. Experience in managing care plans including risks and knowledge of external providers.
About you: Excellent interpersonal and communication skills, with a proven ability to lead with empathy. Someone who values and enjoys working in collaboration with a hands on leadership team. Able to develop our business further.
