Are you a health professional passionate about improving rural and remote healthcare? Western Queensland PHN is seeking a dynamic professional to lead our Healthy Outback Communities initiative, driving health service innovation across Western Queensland.
In this role, you’ll lead a small team delivering culturally safe, community-aligned health and wellbeing services. You’ll work closely with local providers, councils, and community members to embed sustainable health programs that make a real difference.
About the Role
This is a leadership role focused on improving health outcomes in remote communities. You’ll lead a small team of Wellbeing Connectors and work directly with communities to embed culturally safe, locally driven health and wellbeing services. You’ll also coordinate with internal teams and external partners to ensure health programs are sustainable and aligned with broader strategic goals.
You’ll be travelling regularly to communities like Jundah, Windorah, and Boulia, providing on-the-ground support and helping transition pilot projects into long-term, community-owned models.
This critical role will be based within the Central West Queensland region in either Longreach, Jundah, Stonehenge, Blackall, Barcaldine, Winton, Aramac, Boulia, Windorah or surrounding areas.
Key Responsibilities
Provide on-the-ground leadership, supporting Wellbeing Connectors to deliver culturally appropriate and community-aligned services
Co-design health and wellbeing solutions in partnership with community members, providers, and cultural leaders
Oversee HOC initiatives and track outcomes through data and community feedback
Plan travel calendars to deliver timely, on-site support and facilitation.
Prepare briefings, reports, and contribute to cross-sector collaborations
Build the capability of internal teams to sustain HOC outcomes.
About You
Our ideal candidate will have a deep understanding of the Western Queensland. You will also bring:
Experience in health, mental health, or allied health settings
Strong leadership and stakeholder engagement skills
Based in Central Western Queensland
Willingness to travel to communities like Jundah, Windorah, and Boulia
A passion for rural health and community wellbeing
A current driver’s licence and willingness to travel regularly to remote areas.
