Do you have a passion for community development and improving wellbeing in rural areas? Western Queensland PHN is looking for a motivated leader to help shape and deliver community-led health and wellbeing initiatives across the Central West.
You’ll work directly with local communities, councils, and service providers to co-design and implement programs that reflect local priorities and cultural values. This is a hands-on role with real impact.
About the Role
This role is all about empowering communities to lead their own wellbeing initiatives. You’ll work closely with local councils, service providers, and cultural leaders to co-design programs that reflect community priorities. You’ll also support a small team and help transition innovative projects into sustainable services that are embedded in everyday community life.
You’ll be a connector between community needs and strategic health planning, ensuring services are culturally responsive and locally relevant.
This critical role will be based within the Central West Queensland region, for example, Longreach, Jundah, Stonehenge, Blackall, Barcaldine, Winton, Aramac, Boulia, Windorah or surrounding areas.
Key Responsibilities
Provide on-the-ground leadership, supporting Wellbeing Connectors to deliver culturally appropriate and community-aligned services
Co-design health and wellbeing solutions in partnership with community members, providers, and cultural leaders
Oversee Healthy Outback Communities initiatives and track outcomes through data and community feedback
Plan travel calendars to deliver timely, on-site support and facilitation.
Prepare briefings, reports, and contribute to cross-sector collaborations
Build the capability of internal teams to sustain HOC outcomes.
About You
Our ideal candidate will have a deep understanding of the Western Queensland. We’re looking for someone who:
Has experience in community development, health promotion, or social services
Can build strong relationships and lead collaborative projects
Is based in Central Western Queensland
Enjoys working in remote communities and travelling regularly
Has a passion for rural health and community wellbeing
Has a current driver’s licence and willingness to travel to remote areas.
