About Us

We are a family-owned and values-driven NDIS provider, and we're looking for a passionate and experienced Service Delivery Coordinator to join our growing team.
About the Role
As a Service Delivery Coordinator, you'll be the key link between our participants, their support networks, and our support workers. You'll ensure services are delivered safely, effectively, and in line with NDIS standards – all while maintaining a strong focus on advocacy, choice, and control for our participants.
This is a dynamic and rewarding role where no two days are the same. You'll oversee a participant caseload, manage and develop support teams, and play a critical role in ensuring ongoing compliance and continuous improvement across our services.
Key Responsibilities
Develop, implement, and review participant documentation (Service Agreements, PSPs, Risk Assessments, etc.)
Advocate for participants to access funding, supports, and meaningful activities
Build and maintain positive stakeholder relationships (participants, families, coordinators, allied health, etc.)
Manage and review incidents, including staff and participant follow-up
Provide supervision, training, and guidance to support teams, ensuring compliance and quality of care
Lead staff in complex or emergency situations with confidence and professionalism
Support rosters and recruitment within your participant caseload
Maintain compliance with NDIS standards and ensure audit readiness
Work collaboratively with the management team to identify and implement service improvements
Participate in on-call duties, as required
What You'll Bring
Minimum 3 years' experience in NDIS service coordination, case management, or similar disability support role
Minimum Cert IV in Community Services (or equivalent)
Strong understanding of the NDIS framework and quality standards
Excellent communication, leadership, and problem-solving skills
NDIS Worker Screening & Blue Card
First Aid & CPR certification
Current Driver's License, reliable vehicle & comprehensive car insurance
Australian citizenship or permanent residency with unrestricted right to work in Australia
