The Role
We are seeking a passionate and organised Support Coordinator for a permanent position within our Customer Contact Centre team. Reporting to the Support Coordination Practice Manager, this role is pivotal in delivering our services primarily via a telephony model.
You will be responsible for working directly with our Members to implement their NDIS Plans and strengthen their capacity to coordinate their own supports and participate in the community.
What you’ll be doing in the role.
· Working with Leap in! clients to identify suitable supports, programs, activities, and services.
· Obtaining quotes for services and completing referrals.
· Coordinating service providers and supports to meet the individual needs of each client.
· Ensuring service agreements are developed and activated.
· Building each client’s capacity to achieve greater independence and resolve issues that arise.
· Assisting clients to explore and connect with their community.
· Ensuring services are delivered as planned and monitoring clients’ satisfaction.
· Assisting clients to prepare for their NDIS plan reviews , including assessing goals and obtaining necessary quotes.
· Providing reports to clients and the NDIA on outcomes.
· Acting as the key contact for service issues, complaints, and plan reviews.
· Effectively communicating with the National Disability Insurance Agency (NDIA).
· Managing the budget for each support type.

● Excellent interpersonal and active listening skills with the ability to adapt communication styles to various needs.
● Strong organisation, time management, and problem-solving abilities.
● Experience in Support Coordination, with a sound understanding of NDIS processes, budgets, and service agreements.
● A solutions-focused mindset, and a modern view of disability, ready to contribute to a customer-centred environment.
● Ability to develop community connections and foster positive, professional relationships with service providers and stakeholders.
