About the Unit
The Transition to Residential Aged Care (TRAC) Unit is a unique 20-bed ward where acute care nursing meets residential aged care. Our patients have recovered from their acute medical episodes, but during their hospital journey, it’s determined that returning home is no longer the safest option.
The TRAC Unit provides a compassionate and supportive environment for patients and their families as they prepare for the next stage of care. Working closely with patients, families, and carers, our team helps each person transition smoothly and respectfully into aged care facilities, ensuring their medical, emotional, and social needs are met along the way.

About the Role
As a Registered Nurse in the TRAC Unit, you’ll play a pivotal role in supporting patients through a defining moment in their healthcare journey. You’ll deliver comprehensive geriatric and palliative nursing care, manage complex health needs, and provide emotional support to patients and their families as they prepare for aged care placement.
Every day brings a mix of clinical practice, care coordination, and compassionate communication. The Transition to Residential Aged Care Unit provides an exceptionally supportive environment, with dedicated education support available to you five days a week! You will also receive a comprehensive orientation and ongoing access to experienced mentors, educators and nursing leaders to help you build confidence and develop new skills.

You’ll thrive in this role if you are:
Committed to evidence-based, compassionate care and continuous professional development
Knowledgeable and understanding of aging, disabilities and geriatric nursing
Experienced in care of older persons, medical nursing, and dementia care
Genuinely passionate about aged care and person-centred practice
Empathetic and patient, with strong communication and interpersonal skills
Team-oriented, but confident working independently in a dynamic environment
Motivated to learn and grow — junior and early-career nurses are encouraged to apply

Current registration as a Registered Nurse with AHPRA
