Help participants navigate the NDIS, build capacity and achieve their goals. You’ll manage a caseload, coordinate supports, develop and review action plans, connect participants with quality providers, track budgets, and keep compliant records while advocating for participant choice and control.

Coordinate a range of supports to suit clients needs across multiple providers

Coordinate client’s plan to enable greater independence across all domains of their life

Work within funding levels associated with each client (continually track, monitor, and report funding budgets)

Continually gather and document relevant information about clients from other services for the development of evidence-based reports and updates

Liaise and work collaboratively with key stakeholders to establish a mutual goal of coordinating service delivery and developing effective linkages relevant to the achievement of a client’s NDIS goals

You’ll bring

At least 12 months’ experience in NDIS Support Coordination or similar case management

Solid knowledge of NDIS plans, funding streams and the Price Guide

Strong stakeholder engagement, problem-solving and advocacy skills

Excellent documentation and time-management across a dynamic caseload

Proficiency with CRMs and Microsoft 365

Current NDIS Worker Screening Check (or ability to obtain), WWCC if required, and a valid driver’s licence
