Cranbourne West Secondary College is seeking a compassionate, qualified and experienced School Social Worker to join our dedicated Wellbeing Team. This full-time, ongoing role focuses on supporting senior secondary students (Years 11 and 12) and their families to overcome barriers to learning and engagement.
At Cranbourne West Secondary College, the wellbeing team plays a crucial role in supporting students' growth and wellbeing so they can achieve their best academically. The Social Worker will work collaboratively with students, families, staff, and external agencies to promote mental health, strengthen family connections, improve attendance, and support successful post-school transitions.
Key Responsibilities:
Provide individual and group support for senior students experiencing social, emotional, or family-related difficulties.
Conduct psychosocial assessments and develop tailored support plans.
Collaborate with families and external agencies to support student wellbeing and safety.
Deliver early intervention and case management services.
Support students at risk of disengagement or poor attendance with re-engagement strategies.
Facilitate referrals to external services (e.g. mental health, housing, AOD, youth services).
Participate in Student Support Group (SSG) meetings and care team planning.
Assist in managing critical incidents and crisis intervention processes.
Maintain accurate and confidential records in line with departmental and professional standards.
Advocate for students needs within the school and broader community.
Support Year 12 students with planning for transition to further education, training or employment.

SC1 The ability to work as an effective and positive member of a multidisciplinary school wellbeing team.
SC2 Demonstrated understanding and commitment to the development of a comprehensive social work service to students and families throughout a primary school.
SC3 The capacity to develop and implement social work programs as well as facilitate individual and group learning for students, families and staff.
SC4 Demonstrated excellent communication and interpersonal skills including the ability to communicate effectively with a range of professionals, teachers and parents on service delivery and student based issues and to be able to maintain strict confidentiality at all times.
SC5 Demonstrated excellent organisational and time management skills including the ability to meet and exceed deadlines and display proactive and positive initiative and attitude.
SC6 Demonstrated ability to write reports and recommendations to support funding applications for identified students.
