The Role

As a Support Coordinator, you will play a pivotal role in providing exceptional support services to Level 2 and 3 participants.

Your primary responsibilities will include:

Supporting participants to understand their NDIS plans and exercise choice and control.

Connecting participants with supports, negotiating service agreements, and removing barriers.

Assisting with plan reviews, reports, and achieving measurable outcomes.

Building trust and maintaining continuity when stepping into existing caseloads.

Collaborating with stakeholders to strengthen participant networks and resilience.

Upholding Heartland values and person-centred practices.

What We’re Looking For

Relevant tertiary qualification (e.g., Social Work, Community Services) and minimum 2 years’ experience in Support Coordination.

Strong knowledge of the NDIS and disability support sector.

Excellent communication, organisational and problem-solving skills.

Ability to work independently while staying connected to a team.

A genuine passion for making a difference in the lives of people with disability.
