Position: Part Time (3-4 days per week)

Location: Outer South Eastern Suburbs (Dandenong/ Berwick / Cranbourne/ Pakenham areas)

The Role

As a Community Physiotherapist with Agestrong, you will:

Provide assessment, treatment, and rehabilitation for patients post hospitalisation in their homes.

Work with older adults to maintain or improve function, balance, strength and help improve/ maintain their quality of life.

Design and deliver home‑based physiotherapy programs, which may include falls prevention, gait retraining, assistive device prescription & training, and safe mobility practice.

Collaborate with other allied health professionals, hospital discharge planners, aged care providers, and community services.

Maintain clear documentation, outcome measurement, and reporting.

Travel locally between client homes.

Who We’re Looking For

You are someone who:

Has a degree in Physiotherapy, and hold general registered with AHPRA.

May be a new graduate (we welcome and support new grads) or experienced clinician.

Demonstrates excellent interpersonal skills, empathy, and an ability to build rapport with older adults.

Has strong clinical reasoning and ability to work independently in a community/home setting.

Is organised, reliable, and comfortable managing travel between clients.

Holds a valid driver’s licence and access to a vehicle.

Is committed to continuing professional development and evidence‑based practice.
