Your Role as Community Services Worker
You’ll deliver person-centred services across community services. You’ll empower individuals to live more independently, safely, and meaningfully by:
• Providing guidance and support to staff, volunteers and students regarding the facilitation of client participation.
• Providing community, in-home or centre-based social and recreational support to individuals or groups to maintain social connectedness.
• Maintaining client records and undertake timely documentation to report on client progress and reviews.
• Approve services using client data base system to ensure financial compliance requirements.
• Participate in the assessment process of clients, including the development and review of care plans and provide high quality supports in accordance with the individual’s care plan.
• Provide personal support (prompting and assistance with meal preparation and/or personal care) as required to maintain the dignity of individuals participating in services.
• Build positive relationships and communicate appropriately with clients, their families and networks accessing the service.
• Work alongside clients, carer/s, families and others as outlined in their care plans or NDIS plans and in accordance with the WMQ Charter, code of conduct, Aged Care and NDIS standards and WMQ policies and procedures.
• Provide transport for clients utilising BCC vehicles.
• Assist with the daily operations of the centre including responding to enquiries, providing information and referral, assisting with rostering, banking, purchasing, and petty cash.
• Assist with community projects and events and contribute to a newsletter.
What We’re Looking For
Relevant Certificate IV qualification (Community Services, Aged Care, Disability/Mental Health, or equivalent) from a recognised provider for appointment as Level 3; and substantial relevant experience.
Demonstrated ability to provide empathetic, flexible and holistic care to a variety of clients with varying needs including older people or people with a disability.
Highly effective interpersonal and communication skills including the ability to develop rapport and maintain effective relationship with clients, their carers / families and support networks.
Demonstrated ability to work as part of a small team to continually promote and provide excellent care and services to customers.
Computer literacy with Microsoft Office programs and ability to learn database systems and applications.
Willingness to continue to learn new processes and skills and to maintain sound time management.
Demonstrated ability to recognise and report health/environmental hazards including infection control and assist with corrective action.
Demonstrated ability to report changes in a client’s condition, needs or other relevant information to the Senior Coordinator.
Demonstrated understanding and commitment to the principles and practices of privacy, confidentiality and professional boundaries in relation to all clients, volunteers and staff.
Demonstrated knowledge of workplace health and safety regulations and continuous quality improvement practices.
A Valuable Addition, Yet Not a Must-Have!
Relevant degree in a social, community, recreation or health discipline or equivalent experience.
Current Light Rigid (LR) class License or ability to obtain one
Essential Requirements
Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
Relevant probity checks required by legislation and WMQ Limited policy.
Current Driver’s Licence and willingness to travel for work
