About the role
Permanent opportunity to apply Social Work skills and knowledge in the delivery of psychosocial care for Aboriginal and Torres Strait Islander consumers and their families/carers of the Community Palliative Care service. The Social Work service is provided between the hours of 0800 to 1630 hours Monday to Friday, excluding public holidays.
About you
Our ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Deliver professional Social Work clinical services to consumers of the Community Palliative Care service and their families/carers, in accordance with prescribed professional and ethical standards.
- Apply communication, consultative and interpersonal skills to work effectively with consumers, families/carers, multi-disciplinary team members, and key stakeholders, to meet service goals and enhance consumer outcomes particular for Aboriginal and Torres Strait Islander people.
- Actively participate in the identification, development and implementation of quality improvement activities for the delivery of Social Work within the Community Palliative Care service and Social Work discipline.
