About the Role

Compassionate Community Care is seeking a qualified and proactive NDIS Clinical Case Manager – Enrolled Nurse to provide clinical support to our NDIS participants. The role focuses on medication management, clinical compliance, and guiding support staff in delivering safe, high-quality care. Starts as Casual with Full Time hours with potential to become Full Time after 6-month probationary review.

Key Responsibilities

Manage day to day Provider Side operations, including Handovers, Participant Stakeholder meetings, SIL house Management, participant intake processes, participant Activities and General enquiries

Provide after hours On Call support on a rotating roster

Supervise medications in accordance with care plans

Conduct medication audits and ensure accurate documentation (MARs)

Monitor and report changes in participant health

Assist with clinical assessments, care plans, and risk reviews

Maintain accurate clinical notes and progress records

Provide clinical guidance and mentorship to support staff

Liaise with GPs, pharmacists, families, and allied health professionals as needed

Requirements

Current registration with AHPRA (Enrolled Nurse)

Experience in disability, community care, or aged care

Strong understanding of NDIS Practice Standards and frameworks

Excellent communication and documentation skills

Two professional referees (contactable)

NDIS Worker Screening Check (or willing to obtain)

First Aid & CPR certification

COVID-19 vaccination (as required)

Valid Driver’s Licence and reliable vehicle
