About the role

This full-time office based Support Coordinator and Recovery Coach – Level 2 position with Expert Care Services based in Penrith NSW 2750. In this strategic role, you will be responsible for delivering tailored coordination and coaching services to support clients with complex needs as they work towards their recovery and independence goals.

What you'll be doing

Develop and implement comprehensive support plans to address the diverse needs of clients with complex physical, mental health and social support requirements

Coordinate and monitor the delivery of services from a range of providers to ensure clients receive integrated, holistic care

Empower clients to build on their strengths and achieve their personal recovery goals through structured coaching and mentoring

Collaborate with key stakeholders including families, carers and service providers to deliver coordinated, person-centred support

Maintain detailed client records and produce comprehensive reporting to track progress and outcomes

Provide leadership, guidance and development to more junior members of the support coordination team

What we're looking for

Tertiary qualification in Social Work, Occupational Therapy, Psychology or a related discipline

Extensive experience in a support coordination or recovery coaching role, preferably in the complex care, disability or mental health sectors

Strong understanding of the NDIS and experience navigating the complex care and disability service systems

Demonstrated ability to develop and implement tailored support plans that address the holistic needs of clients

Excellent interpersonal and communication skills to effectively liaise with a wide range of stakeholders

Commitment to empowering clients and promoting their independence, choice and control

Ability to work autonomously as well as collaboratively within a team environment
