About the Role
We’re seeking an experienced and compassionate Suport Coordinator to join our growing team.In this rewarding role, you will work directly with NDIS participants to coordinate their supports, strengthen their capacity, and connect them with services that best meet their needs.
This Role is ideal for someone who thrives on mentoring others, building and growing a team round, managing complex coordination cases, and driving best-practice participant outcomes.
Key Responsibilities:
Help participants understand, implement, and manage their NDIS plans.
Connect participants to community, mainstream, and funded supports.
Coordinate service delivery and maintain strong relationships with providers and stakeholders.
Support participants to develop independence, confidence, and self-direction.
Monitor goal progress and assist in preparation for plan reviews.
Maintain accurate, timely documentation and case notes.
Work collaboratively with team members to ensure a high standard of service delivery.
This role is primarily community-based, with flexible options to work from home and the office as needed.

About You
You are compassionate, motivated, and committed to empowering people with disability to live fulfilling lives. You bring a strong understanding of the NDIS and a passion for helping others succeed.
Essential Criteria:
Experience as a NDIS Support Coordinator
Minimum Diploma in Community Services, Disability, Mental Health, Social Work, or related field.
Strong communication, organisation, and administrative skills.
Current Driver’s Licence and access to a reliable vehicle.
Valid NDIS Worker Screening Check and Working with Children Check (or willingness to obtain).
Completion of the NDIS Worker Orientation Module: “Quality, Safety and You.”
Desirable:
Knowledge of the Hunter and Central Coast regions’ community and disability networks.
Experience supporting participants with psychosocial or complex physical physical needs.
