Care Worker (Qualified) - Aged Care Sector

Permanent Part Time | 15 to 45 hours per fortnight (subject to your availability) | Workplace: On Site | $34.59 per hour

About Us

South West Community Care is a not-for-profit Organisation and Registered Charity local to the South West, providing aged care and in-home services to the local community. It is managed by a Chief Executive Officer (CEO) who is supported by a Chief Governance Officer and Management Team overseeing operations and supporting a team of over 120 employees and volunteers. The CEO, in conjunction with a volunteer Board of Management, sets the strategic direction for the organisation.

South West Community Care is managed locally and embraces the ‘Wellness’ Approach when delivering care to eligible older Australians in the Community approximately 1000 clients currently, providing services designed to maintain independence dignity and quality of life for our ageing community. These services are delivered by compassionate and caring staff working within the guidelines set out by the Aged Care Charter of Rights and Code of Conduct for Aged Care to deliver person centred care to optimise positive outcomes.

Position Purpose

Reporting to the Workforce Coordinator, the purpose of your role is:

  • Assist our clients to retain or regain independence.
  • Encourage our clients to do as much as possible for themselves.
  • Promote and monitor our clients’ overall wellbeing.
  • Read, understand and follow the directions in the client’s care plan, with regard to providing care and support to the client.
  • Position Responsibilities
  • As a Care Worker you will be matched based on skill level, personality traits and preferences to support long-term relationships.
  • As typical in aged care support, you will be assisting with:
  • Social Support: joining our clients for a morning walk or having a chat in their home.
  • Domestic assistance: supporting with everyday tasks around the home.
  • Personal Care: ensuring our clients live well and with dignity at home.
  • Transportation: to appointments, friends’ houses or out shopping.
  • Medication reminders: keeping seniors in their homes and out of hospital; and other activities of daily living.

To thrive in this role you will have:

  • Certificate III or IV in Individual Support/Aged Care/Disability Support.
  • A HLTAID011 Provide First Aid statement of attainment.
  • HLTHPS006 Assist Clients with Medication statement of attainment.
  • The ability to be flexible and adaptable to meet clients’ individual needs.
  • Strong communication and organisational skills.
  • Own a smart phone with a sufficient data plan (Communications Allowance paid fortnightly).
  • Current WA full driver’s license and a registered and insured vehicle – travel is part of your role and you will be paid for travel time between clients.
