Your new role:
As a Social Worker at Princess Alexandra Hospital, you will deliver high-quality, person-centred care, supporting patients and families through assessment, intervention, education, and safe discharge planning.
In this role, you will:
- Deliver comprehensive psychosocial assessments with a focus on trauma-informed care, risk management, and discharge planning.
- Navigate NDIS and My Aged Care pathways, staying up to date with aged care legislation and reforms.
- Collaborate with interprofessional teams to ensure patient-centred outcomes and seamless service delivery.
- Maintain accurate documentation, manage your caseload effectively, and report service statistics.
- Contribute to quality improvement and research activities within the Social Work department.
- Provide clinical education, guidance and supervision for students and junior staff.
About you:
You are a compassionate, skilled Social Worker who thrives in a dynamic hospital environment and is committed to delivering safe, equitable and evidence-based care. You bring:
- Tertiary qualification in Social Work and eligibility for AASW membership (or AASW certification of eligibility if overseas qualified).
- Experience in hospital-based social work or a similar health setting, with strong skills in psychosocial assessment and discharge planning.
- Knowledge of NDIS, My Aged Care systems, and current aged care reforms.
- Excellent communication, negotiation, and conflict resolution skills for working collaboratively with patients, families, and teams.
- Ability to manage competing priorities and maintain accurate documentation.
- Commitment to professional growth, continuous learning, and quality improvement initiatives.
- Current Blue Card (Working with Children Check) or ability to obtain one prior to appointment.
