The First Nations Patient Journey Hub (FNPJH) is a new service within the Indigenous Health Service Division at Townsville University Hospital. It supports culturally responsive care transitions for First Nations patients from hospital to community.

- Salary range: $3777.60 to $4453.50 p.f. (HWF4)
- Annual leave with 14%-17.5% loading
- 12.75% employer contribution to superannuation

About your opportunity
This is an exciting opportunity for a Senior Aboriginal & Torres Strait Islander Health Worker (Identified) to permanently join the Townsville Hospital and Health Service (HHS) ‘First Nations Patient Journey Hub' (FNPJH). This new service facilitates a culturally capable and responsive health care journey for First Nations patients from the Townsville University Hospital (TUH) to community care pathways.
Requirements:
- Mandatory qualifications (Diploma of Aboriginal and/or Torres Strait Islander Primary Health Care) and proof of registration (if applicable).
- Under s25 of the Anti-Discrimination Act 1991, there is a genuine occupational requirement for the incumbent to be Aboriginal and/or Torres Strait Islander to work in the to the Aboriginal or Torres Strait Islander Community.
- Possession of class C drivers' licence.
- This is a Vaccine Preventable Disease (VPD) risk role. Hepatitis B, Measles, Mumps, Rubella, Varicella, Pertussis and Tuberculosis.
- Probation period may apply.
- Pre-employment checks will be conducted including criminal history and Blue Card checks.
