Friends Home Care Nursing Services Pty Ltd are looking for an Experienced Registered Nurse with a vast knowledge of working with Acquired Brain Injury in Community Home Care. To be a successful applicant you will need to be a team player with behaviour management skills, effective communication skills, patience and empathy, and the ability to prioritise the needs of the client. There will be onsite training and preceptorship should you be successful in this role. Minimum of 3yrs experience in a challenging environment, the role is primarily based in Lilyfield NSW 2040.

Duties include, but not limited to:

Conduct physical, functional, and psychosocial assessments.

Monitor vital signs and symptoms of illness or deterioration.

Identify risks (e.g., falls, pressure injuries, malnutrition).

Behaviour management and client engagement.

Administer prescribed medications (oral, subcutaneous, intramuscular, IV if required).

Monitor for side effects or interactions.

Assist with medication reviews and liaise with pharmacists/GPs

Assess, clean, and dress wounds (acute, surgical, chronic, pressure injuries).

Monitor healing progress.

Educate clients and carers on wound management.

Maintain up-to-date, professional clinical documentation.

Record assessments, interventions, outcomes, and incidents.

Use digital systems (e.g., My Aged Care, home care provider portals).

Identify and report hazards, incidents, and client deterioration.

Implement infection control and safe manual handling practices.

Monitor home environments for safety issues.

Educate clients and carers on:

Medication use

Chronic disease management

Nutrition and hydration

Continence management

Preventing hospital readmissions

Participate in staff training, audits, and quality improvement activities.

Support and mentor Enrolled Nurses (ENs) and care workers.

Ensure care is culturally safe and respectful of diversity.

Adhere to the Nursing and Midwifery Board of Australia (NMBA) standards.
