The Care Partner – Aged Care Programs, is responsible for accepting referrals, arranging and facilitating the delivery of in-home services and supports for people assessed as eligible to receive Commonwealth Home Support Program (CHSP) services or Fee-for-Service programs (DVA – Veterans Home Care and Home and Community Care Program for Younger People [HACC-PYP]).
In this rewarding role you will:
Ensure that older people and their carers are supported to maximise independence, wellbeing and participation in community life, while remaining safe and well at home
Contribute to all aspects of program operations within contractual requirements, achieving KPIs and targets
Accept and process referrals in line with funding program guidelines, ensuring timely access to services

This is a permanent full-time role based out of any of our NSW Aged Care Services offices. These offices include Albury, Corowa, Deniliquin, Finley and Griffith.
This position has been classified under the Social, Community, Home Care and Disability Services Award as Level 4 with a starting annual salary of $88,090 + Superannuation + Salary Packaging.
What we’re looking for
A minimum two years’ experience in home-based aged care
Understanding of care coordination, consumer-directed care and person-centered practice, within a wellness and reablement framework
Strong time management and organisational skills, with the ability to manage competing priorities
