The opportunity
You will play a pivotal role in supporting participants to maintain their independence and achieve their individual goals. This includes conducting comprehensive assessments to identify care needs and developing, implementing, and reviewing tailored Care Plans in close collaboration with consumers and their families. You will provide ongoing case management for Home Care Package (HCP) participants, ensuring that services remain effective, compliant, and aligned with wellness outcomes. Key responsibilities include overseeing HCP budgets, maintaining accurate documentation to meet all regulatory and organisational requirements. Building strong and respectful relationships with participants, carers, health professionals, and community partners is essential to ensure seamless service delivery.
You will bring demonstrated experience in care planning and the financial management of HCP budgets, together with strong organisational, administrative, and problem-solving skills. Excellent communication abilities, both written and verbal, are essential, along with a genuine commitment to working with and supporting older people to live well.
Essential Requirements
National Police Clearance, or willingness to obtain
Current Drivers Licence
Appropriately insured motor vehicle
Tertiary qualification in Aged Care, Community Services, or Nursing
Demonstrated experience in case management and community sector
Strong interpersonal skills with the ability to build positive, respectful, and collaborative relationships
Proven ability to interpret and monitor budgets, with excellent written communication skills to prepare clear reports and accurate documentation.
