About the Opportunity

NVC Group currently has a fantastic opportunity for a Home & Community Care Coordinator to join our dedicated team.

In this role, you will oversee and coordinate a range of community programs, including Home Care Packages (HCP), Commonwealth Home Support Program (CHSP), Flexible Care Packages, Private Home Support, and Brokerage Services. You'll work closely with clients, their families, and service providers to ensure the delivery of high-quality, person-centred care and support within a collaborative team environment.

This is an immediate start opportunity based at our Coffs Harbour Office.

What You'll Bring to the Role

Minimum qualification of Certificate IV in Aged care, Community Services or a related field.

Extensive experience in Home Care Packages (HCP), Commonwealth Home Support Program (CHSP), Flexible Care Packages, Private Home Support, and Brokerage Services.

Excellent written and verbal communication skills, with the ability to build and maintain strong community and professional networks.

Sound computer literacy and administrative capability.

Proven ability to coordinate services for clients with complex needs and collaborate with multiple providers.

Strong organisational and multitasking skills, with the ability to manage competing priorities effectively.

Self-motivated with the ability to work independently and demonstrate initiative.

Commitment to professional growth and ongoing learning.

Current Driver's Licence.

Current First Aid Certificate and National Police Check (or willingness to obtain).

Working with Vulnerable People (WWVP) clearance is required before commencement.

Australian citizenship is required for this position.

Desirable

Tertiary qualifications in Health Care, Aged Care, or Management.

Membership or involvement in relevant Industry Reference Groups, Committees, or Professional Associations.
