The Opportunity:

As one of the Clinical Care Coordinators of this 100+ bed Nursing Home in Sydney’s South-West, you will play a key role in the clinical oversight of this modern Nursing Home.

• Your main responsibility will be leading quality clinical care for residents across a small section of the home (30-40 residents), while promoting a positive culture within the Nursing/Care team.
• You will play a key role in coordinating communication and services between residents, families, and relevant medical professionals.
• You will ensure quality improvement initiatives are implemented.
• You will work under the lead of a Care Manager, together striving for excellent clinical standards and care for residents.

Key Information:

• Salary package in the region $120,000 (inclusive of superannuation).
• Large Nursing Home in Sydney’s South-West - 100+ beds. This is a well-supported site with Clinical Care Coordinators allocated to each small section of the home, with a Care Manager supporting the whole site from the top.
• You will report to the Care Manager, who reports to the Facility Manager on site (clinical). Within the onsite structure, this position sits as 3rd in charge.
• Modern home with fantastic amenities.
• The home passed its most accreditation and is due their next in 2027.
• This home is owned by a well-established organisation in the Sydney market with a strong staff retention record.

Requirements:

• AHPRA Registered Nurse - excellent clinical skills and knowledge.
• At least 3 years experience working as a Registered Nurse in Australia, inclusive of Residential Aged Care experience.
• Prior Leadership experience in a CCC/CM role preferred. Acting CM experience / RNs in Charge also invited to express interest.
• A thorough knowledge of the Aged Care standards, ANACC, and the Accreditation process.
• Ability to lead, mentor and drive excellent results from the team, with a focus on raising the standard of resident care.
