Support Coordinator – Full Time

Location: Campsie, NSW

About the Role

Join the growing team at Affective Care as a Support Coordinator in a rewarding full-time position based in Campsie. In this role, you’ll work closely with people living with disability, helping them access the services and resources they need to live with independence, dignity, and choice.

What You’ll Be Doing

Develop person-centered support plans in collaboration with clients and their families

Coordinate and monitor the delivery of services to align with client goals and budgets

Advocate on behalf of clients to the NDIS in order to secure appropriate funding, equipment, and resources

Build and maintain strong relationships with a network of community service providers

Manage a case load of approximately 40 clients.

Provide emotional support, guidance, and mentoring to empower clients towards independence

Regularly review and update support plans to meet changing client needs

Maintain accurate case notes and ensure compliance with reporting requirements

What We’re Looking For

Minimum 2 years’ experience in support coordination or case management

Strong knowledge of the disability and aged care sectors, including funding models (e.g., NDIS)

Excellent communication and interpersonal skills with a client-focused, empathetic approach

Highly organised, with proven skills in care planning, service coordination, and stakeholder management

Ability to work independently and collaboratively within a team environment

Proficient in Microsoft Office and client record management systems

Current driver’s licence and access to a reliable vehicle preferred
