About the Role

As the Nurse Coordinator, you will be responsible for coordinating patient care, mentoring nurses, reviewing documentation, and ensuring high clinical standards across our services. This is a hands-on leadership role that involves collaborating with rostering, intake, support coordination, case managers and management teams to deliver seamless, person-centred care for our clients.

Key responsibilities include:

Coordinating and reviewing patient care plans and progress

Overseeing nursing documentation, continence assessments, and clinical compliance

Providing guidance, mentorship, and training to nursing staff

Leading quality assurance checks, auditing notes, and ensuring regulatory compliance

Liaising with GPs, case managers, and allied health professionals

Supporting continuous improvement initiatives to enhance client satisfaction

What We’re Looking For

Registered Nurse (RN) with current AHPRA registration

Demonstrated experience in patient care coordination and team leadership

Excellent written and verbal communication skills

Strong organisational skills with the ability to prioritise and manage multiple tasks

A collaborative approach and ability to work with interdisciplinary teams
