The Role
This is a 6months FTC position, Monday to Friday.
Our Care Partners lead the planning, coordination, and monitoring of individualised care management across a range of Benetas services, including Support at Home, Veterans Home Care, respite services, and private in-home care. The Care Partner works in partnership with older people, their families, and others to ensure care is safe, responsive, and aligned with each person’s goals, needs, and preferences. Some of the role responsibilities include:
- Performing assessments and co-creating care plans with older clients and their representatives, to support client autonomy, informed decision-making, and respect for dignity of risk
- Coordinating service access, including referrals, scheduling, and liaison with providers
- Helping clients manage budgets, authorise spending, and interpret statements
- Tracking service quality and timeliness and addressing any gaps proactively. Monitoring changing needs, updating plans, and escalating when necessary
- Acting as the main contact for clients and families—providing advocacy and problem-solving
- Maintaining accurate documentation and compliance with aged care regulations and funding programs
- Participating in risk escalation, safeguarding, incident reviews, audits, and quality improvement initiatives
About you:
- Qualifications in allied health, community health or nursing
- Demonstrable experience in case management of government funded packages, community resources, and relevant legislation in the provision of aged care services
- Knowledge and experience in best practice quality models of assessment, care plan development, implementation, and evaluation
- Exceptional interpersonal and communication skills to demonstrate empathy and understanding to older clients and their families
- A positive and proactive approach to working collaboratively within a team and the broader community
- Ability to use initiative and work under pressure
