The Single Aged Care Assessment System is a multi-disciplinary team providing assessment services to older Queenslanders in the community. The assessment system aims to simplify and improve access to aged care services. - Salary range: $76,963 to $85,591 p.a. (AO3)
- Annual leave with 14%-17.5% loading
- 12.75% employer contribution to superannuation
- Plus Salary Sacrificing options may be available
About your opportunity:
As a Home Support Assessor, you will provide non-clinical home-based face to face assessments of clients referred via the My Aged Care Gateway using the Integrated Assessment Tool (IAT). Under the Single Aged Care Assessment System, Home Support Assessors will work with clients and their representative/s to establish goals specific to the client's needs and develops appropriate strategies and actions, including referrals to aged care and non-aged care services.
Requirements:
- Certificate III in Home Care or Aged Care (mandatory).
- Certificate IV in Health Service Assistance/Allied Health Assistance (highly desirable).
- Possession of class C drivers' licence.
- This is a Vaccine Preventable Diseases (VPD) risk role.
- A Probation period may apply.
- Pre-employment checks will be conducted including criminal history clearance.
